Medication Forms

To administer students' medication, we need to follow policies set out by the Department of Education - please see the Parent Information Brochure for more information. 

Please ensure that you are aware of Expiry dates on Medication stored at school and provide replacements when required.

If any of your child's Health Information has changed, please complete the Student Health Care Summary;
